How The Health Collaborative impacts San Antonio's shared Community Vision

The Health Collaborative (THC) is the source for health data in Bexar County, through the Community Health Assessment and Community Health improvement Plan, two data-drive communications provided to track and compile health data from such disparate sources as the Center for Disease Control, San Antonio Metropolitan Health District, all major hospital systems, HASA, and more. THC's data is what the hospital systems use to plan their community health outreach and activities.
